Community Action Plan
Background
The campaign has been tracking successfully on a number of KPI’s, and at the halfway point, a symposium was held to celebrate these achievements, and set the scene for the campaigns future. It was agreed that a Community Action Plan would benefit the campaign and to continue the success of the campaign beyond the 3 year commitment.
A workshop was held on the 15th October 2015 with participating 500 Lives 500 Homes partners, facilitated by Jessica Venegas Director of Strategic Partnerships for Community Solutions in the USA. It was during this workshop, and reflecting on the campaign to date, and by doing a SWOT analysis, that a Community Action Plan was identified as needed and was agreed by the Planning Group.
At the recent 500 Lives 500 Homes monthly planning session, the foundations for a Community Action Plan consultation was commenced, by reviewing the SWOT analysis conducted at the 15th Oct workshop. The discussions and agreements from this session has been captured in the attached template, proposing to be used as the basis for the Community Action Plan report.

Why a Community Action Plan
A Community Action Plan is a road map for implementing community change, by identifying and specifying WHAT will be done, WHO will do it and HOW it will be done. In other words, the action plan describes what the community wants to accomplish, what activities are required during a specified timeline and what resources (money, people and materials) are needed to be successful. The community action plan becomes a framework for implementing change in a community.
Why get involved
The 500 Lives 500 Homes campaign has been a very successful campaign to date, with plenty more to achieve and deliver for the remainder of the campaign and beyond. The current format of the campaign needs to be strengthened and reinforced – with more resources, improved processes, greater participation and more housing supply. The Community Action Plan is a way of addressing these challenges, and setting the path for campaign success as well as a mechanism to contribute to the long term systems change needed in homelessness in Brisbane.
Objectives
By developing a framework and roadmap for the campaign to end homelessness, current and new partners will have clear guidance on their contribution and impact. Attracting additional resources will also be possible as the momentum increases. The Infographic of the 500 Lives 500 Homes campaign forms the basis of these objectives:
- inform all existing and potential stakeholders, partners and supporters of the success of the campaign to date, illustrating that we know what the problems are, and that we have solutions and goals for the campaign identified;

Process and timeframes
The community consultation to inform the Community Action Plan will be independently facilitated by Social Scaffolding to achieve agreement from stakeholders on WHAT will be done, WHO will do it and HOW it will be done. A report will then be distributed to attendees, stakeholders, and the broader community.
